- Aircraft was with 6th Recon Squadron.
- Engine failed in flight, resulting in a forced landing. The aircraft landed heavily and
nosed over. Both occupants survived, but the aircraft was written off.
- The engine was salvaged and used in the reserialled DH9A #65.
